Introduction

Quetiapine fumarate (quetiapine) is a dibenzothiazepine derivative marketed as an immediate-release formula and has proven efficacy and safety in the treatment of patients with acute or chronic schizophrenia (Emsley et al., 2000). Furthermore, quetiapine has been reported to be effective in patients with bipolar mania in a controlled trial (Sachs et al., 2004). Quetiapine has few side-effects on both the neurological and endocrinological systems, and also has prominent advantages when administered to the elderly and woman (Correll et al., 2004). However, the antihistaminic and alpha-1 adrenergic properties of quetiapine, which lead to somnolence and orthostatic hypotension, respectively, may require initial titration periods. Recently, the effect of quetiapine on the acute agitation was found to be superior to haloperidole-treated patients in the treatment of behavioural agitation among acutely exacerbated patients with schizophrenia (Chengappa et al., 2003a).

quetiapine in the treatment of acutely ill bipolar disorder. This preliminary study proposes that rapid titration of quetipaine in combination with a mood stabilizer for the treatment of bipolar mania is effective and tolerable in comparison with conventional titration. A controlled study with a larger cohort should be performed. Int Clin Psychopharmacol 20:327–330 c 2005 Lippincott Williams & Wilkins.
